I'm Maksim Birikov, DPT. The physical therapist Baltimore books by name.

Physica Medica is Baltimore's only concierge physical therapy practice. Every session is a full 60 minutes, one-on-one with me. No rotating therapists, no aides, no fifteen-minute hand-offs. Manual therapy and movement rehabilitation, integrated by one practitioner, in one studio in Fells Point.

The Concierge Model

One-on-One Physical Therapy in Fells Point, Baltimore, With Maksim Birikov, DPT

You know how a typical PT clinic works: a rushed evaluation, a few minutes of hands-on treatment, then you're handed to an aide who watches you do clamshells next to three other patients. You come back twice a week for months, see a different therapist half the time, and the problem you came in with is still there.

I built Physica Medica to be the opposite of that. I am the only practitioner here. When you book a session, you get me for the entire hour, every visit. I do the evaluation, I do the manual therapy, I coach the corrective work, and I adjust the plan because I actually remember what your body did last week.

My approach integrates hands-on treatment (dry needling, myofascial cupping, IASTM, deep tissue and sports massage) with movement rehabilitation in the same session, under the same set of hands. Most clinics silo those two things. Splitting them is why results don't hold. Releasing a trigger point without retraining the movement pattern that created it is a two-day fix. I'm not interested in two-day fixes.

What I Do

Three Disciplines. One Practitioner. One Plan.

Everything at Physica Medica works as an integrated system, not a menu of disconnected services.

Manual Therapy

Dry needling, myofascial cupping, IASTM, scar tissue mobilization, and deep tissue and sports massage. Precise, hands-on work that releases trigger points, breaks up adhesions, and restores tissue mobility. Clinically driven, not a spa service.

Movement & Rehab

Strength training, posture re-education, and pregnancy physical therapy. This is where results become permanent: retraining the patterns behind your back pain, forward head posture, or recurring injuries so they stop coming back.

Specialty Programs

Pancafit postural stretching, non-surgical scoliosis correction, diaphragmatic rehabilitation, therapy for singers, and the Wim Hof Method. Techniques I trained in Europe to bring here. You will not find them at another Baltimore clinic.

Services That Don't Exist Anywhere Else in Baltimore

01

The Only Pancafit Class in the United States

Pancafit is an Italian method of global postural realignment, decompensated stretching that releases the entire fascial chain, not one muscle at a time. I trained in it in Europe and teach the first and only class in the US, right here in Fells Point. If your hunched back or tight hips have survived years of ordinary stretching, this is why.

02

Scoliosis Correction Without Surgery or Braces

A structured, movement-based program that measurably improves spinal alignment in adolescents and adults. No hardware, no bracing, no waiting to see if it gets worse. Most patients are told their only options are watching and operating. There is a third option, and I built my practice around it.

Baltimore's First Certified Wim Hof Method Instructor

Breathwork and cold exposure with a measurable physiological mechanism: down-regulating a nervous system stuck in fight-or-flight, which is where chronic pain and chronic tension live. I was the first certified Wim Hof instructor in Baltimore, and I integrate the breathing work directly into rehabilitation, not as a wellness add-on.

What I Treat

Conditions We Treat

Chronic pain, muscle knots, numbness and tingling, recurring injuries. If it involves how your body moves, it's my territory.

Who This Is For

Who We Serve

My patients are smart, health-conscious professionals who have already tried the obvious options. The chain PT clinic that billed insurance for group exercise. The chiropractor whose adjustments lasted three days. The spa massage that felt great and fixed nothing. They're done with generic. They want the problem actually solved.

If you're in Fells Point, Canton, or Harbor East, you're minutes away on foot. Patients also come from Federal Hill, Butchers Hill, and Patterson Park, and from across Baltimore when they've run out of other options. This is a walkable, boutique practice built for people who value their time: one appointment, one hour, one practitioner, zero waiting room churn.

And yes, one-on-one concierge physical therapy costs more per session than a copay. It also takes far fewer sessions, because every minute of every visit is treatment. Most patients find that's not a premium. It's the math.

Questions Worth Asking

Three things no one else in this market can say. Every session is one-on-one with me, Maksim Birikov, DPT, the same practitioner, every visit, for a full hour. I integrate manual therapy and movement rehabilitation in the same session instead of siloing them. And I offer techniques with zero local competition: the only Pancafit class in the US, non-surgical scoliosis correction, diaphragmatic rehabilitation, therapy for singers, and Wim Hof Method instruction. Chains can't replicate this model. That's the point.

Physica Medica is a cash-pay concierge practice, and I won't apologize for that. It's what makes the model work. No insurance bureaucracy dictating your visit count, no 15-minute treatment windows, no aides. You get a full 60 minutes with a Doctor of Physical Therapy, every time, which is why most patients need fewer total sessions. HSA and FSA funds are typically eligible, and I can provide documentation for out-of-network reimbursement. Full details are on the Insurance & Pricing page.

Call or text (443) 228-8029, or book online through the contact page. Your first visit is a full assessment, an hour with me, not a form and a hand-off. If you're not sure whether you need massage, dry needling, or physical therapy, don't worry about picking. That's my job. Read what to expect at your first visit.

Ready to Book? Here's How to Reach Us

Let's be direct about cost, because it's a fair question. Physica Medica is a cash-pay practice. The price buys a full 60-minute session with a DPT, not 15 minutes with an aide. HSA and FSA funds are typically eligible. And because every minute is real treatment, you'll likely need fewer sessions than a traditional clinic would schedule. No endless plans, no lock-in. We work until the problem is fixed.
